Client Billing Policies
Here at Amavi Therapy Center, we strive to make the payment process as seamless as possible. Below you will find details about our accepted insurance plans, payment methods, and billing policies.
Accepted Insurance Plans
We are in-network with the following insurance providers:
United Healthcare
Optum
Aetna
UMR
If you have coverage with one of these providers, we will submit claims directly on your behalf. However, we recommend verifying your benefits and coverage with your insurer before your appointment.
Self-Pay and Out-of-Network Coverage
If you are uninsured or your insurance plan is out-of-network, we offer competitive self-pay rates. Payment is due at the time of service. We can also provide a superbill upon request for potential reimbursement from your insurance provider.

Credit and debit cards (Visa, MasterCard, American Express, Discover)
Health Savings Account (HSA) and Flexible Spending Account (FSA) cards

Account Standing
If your payment account reflects 3 or more past due payments, your therapist reserves the right to cancel all future sessions, until payment has been made. Please notify your therapist prior to your sessions, if alternate payment arrangements will need to be made.
Cancellation and No-Show Policy
To provide the best care to all our patients, we require a 24-hour notice for cancellations or rescheduling. Late cancellations and no-shows will be subject to a fee.
